Following the issue of DSCN 07/2004 'Data Standards: Meta Model', the NHS Data Model and Dictionary was changed to reflect and be based upon a more generic logical data model which better supports the strategic way forward.Following the issue of DSCN 07/2004 'Data Standards: Meta Model', the NHS Data Model and Dictionary was changed to reflect and be based upon a more generic logical data model which better supports the strategic way forward. This NHS Data Model and Dictionary was published as the NHS Data Model and Dictionary Version 3.
There was extensive quality assurance of Version 3 including involvement of the Data Definition Group. All Version 3 contents were cross mapped and checked to ensure consistency with the Version 2 contents. Version 3 superseded Version 2 on 1st May 2005.
Version 3 supports all the messages, data sets and central returns supported by Version 2 but the underlying structure was genericised. No changes were made to NHS Business Definitions.
Version 3 introduced a new Generic Model which is 'PERSON based' rather than 'ORGANISATION based'. The model has been developed around generic 'CARE ACTIVITY' for a PATIENT at 'SERVICE POINT' facility or LOCATION with each event transaction being recorded. This has involved grouping many of the old classes into generic classes like ACTIVITY GROUP. However, none of the detail or approved definitions or value sets in Version 2 have been lost although some of the old class definitions will now be found in the new 'NHS Business Definitions'.
The NHS Data Dictionary Version 2 has been frozen and made available for archive information purposes only and has been watermarked accordingly. All changes to NHS information standards and the supporting Data Set Change Notices are now consistent only with Version 3.
Summary of main changes and enhancements incorporated within Version 3
- Introduction of the capability of holding Retired Items
- Introduction of NHS Business Definitions, which allow specific business areas to be defined in a more flexible manner
- New ‘All Items Index’ which lists all the contents held within the NHS Data Model and Dictionary
- Reduction in the number of data model diagrams making them more comprehensible
- Introduction of Domains, which are conceptual logical modelling objects which identify the logical format, length and value set attributable to one or more attribute(s)
- 'Where Used' list expanded to include a description of usage column
- Creation of a separate Meta Model area within the publication
The introduction of Version 3 had no impact on current information standards or system suppliers.
About the Generic Model
The Generic Model supports all the messages, data sets and central returns which the model supported but the underlying structure has been genericised. This involved grouping many of the old classes into generic classes like ACTIVITY GROUP. However, none of the detail was lost and most of the old class definitions can be found under 'NHS Business Definitions'. The number of diagrams has been greatly reduced and they now describe coherent areas of the model which is a far more useful approach for data modellers.
This has led to the creation of a new Generic Model which is 'PERSON based' rather than 'ORGANISATION based'. The model has been developed around generic 'CARE ACTIVITY' for a PATIENT at 'SERVICE POINT point' facility or location with each event transaction being recorded.
The model is aimed at enabling all 'CARE ACTIVITIES' related to the same condition for the same PATIENT to be recorded and linked across ORGANISATIONS. The different states of the same 'CARE ACTIVITY' are recorded as event transactions e.g. requested, intended, scheduled, provided, cancelled, etc. These different state events also drive the scheduling and capacity planning of resources (people, equipment, facilities, etc.) to deliver the care.
Although this may seem complicated, it is necessary both to form a coherent logical model and to relate physical information such as that which flows on the Data Sets to the logical model. Every physical item should be represented logically in the NHS Data Model and Dictionary. However, the scope of the logical model is greater than the physical information it holds and therefore not all logical information has a physical existence.

Population Validation
All Data Elements are subject to length validation. Some Data Elements are also subject to format and content validation against a list of permitted values defined in the NHS Data Model and Dictionary. The value lists are held on the Attribute which the Data Element is based on, plus default codes which are held on the Data Element itself.

Hospital Episode Statistics is the data source for a wide range of healthcare analysis for the NHS, Government and many other ORGANISATIONS and individuals. It contains admitted patient care data from 1989 onwards, with more than 15 million new records added each year, and outpatient attendance data from 2003 onwards, with more than 60 million new records added each year.

A Learning Disability (formerly known as a Mental Handicap and referred to as an Intellectual Disability) is a type of DISABILITY.
A Learning Disability usually has a significant impact on a PERSON's life. A PERSON with a Learning Disability finds it harder than others to learn, understand and communicate.
People with Profound and Multiple Learning Disabilities need full-time help with every aspect of their lives, including eating, drinking, washing, dressing and toileting etc.
Someone is considered to have a Learning Disability when they function at a level of intellectual ability which is significantly lower than their chronological age. This is usually considered to be equivalent to having an Intelligence Quotient (IQ) of seventy or less:
- Mild Learning Disability (roughly equivalent to an IQ of fifty to seventy) is comparable to the educational term 'Moderate Learning Difficulty'. It is usually caused by a combination of restricted learning and social opportunities plus a high rate of low to average intellectual ability and Learning Disability in close relatives.
- Moderate-to-profound Learning Disability (roughly equivalent to an IQ below fifty) is comparable to the educational term 'Severe Learning Difficulty'. It usually has a specific biological cause.
Note: A Learning Difficulty is a type of Special Education Need which affects areas of learning, such as reading, writing, spelling, mathematics etc.

The NHS National Chlamydia Screening Programme (NCSP) is a Screening Programme.
The aim of the NHS National Chlamydia Screening Programme is to detect undiagnosed chlamydia infection through proactively offering screening to all sexually active young people aged under 25, in a variety of health and community settings, in order to reduce the population prevalence of Chlamydia and prevent the development of associated health problems.

The Ophthalmic Qualifications Committee is delegated by the Department of Health to oversee applications by those who seek admission to the Central Professional List of Medical Practitioners authorised to work in the General Ophthalmic Services as an OPHTHALMIC MEDICAL PRACTITIONER.

A Pharmacy Premises is a location where medicinal drugs are dispensed and sold.
General Pharmaceutical Council Registration of Pharmacy Premises:
The General Pharmaceutical Council (GPhC) maintains a register of Pharmacy Premises if the principal activity at the premises is the retail sale or supply of Pharmacy (P) medicines and / or Prescription Only Medicines (POMs). They are not primarily concerned with NHS Prescriptions.
Once a Pharmacy Premises is registered with the General Pharmaceutical Council it is allocated a registration number which uniquely identifies each registered Pharmacy Premises. This registration number is a 7 digit number starting with the number 1.

To be able to dispense NHS Prescriptions, the Pharmacy Premises requires an Organisation Data Service code in addition to the General Pharmaceutical Council registration number.

A SNOMED CT Subset is a set of SNOMED CT® (Systematized Nomenclature of Medicine Clinical Terms) Concepts, Descriptions, or Relationships that is appropriate to deployment to support particular requirements of implementation.
SNOMED CT Subsets support user interface development through the organisation of clinical display, creation of menus and pick-lists, or support of knowledge structures.
SNOMED CT Subsets may be created as value sets for messaging or data entry or to provide pre-coordinated SNOMED CT® concepts for certain constrained use, or can inter-relate with qualifier value tables appropriate to the concept types.
SNOMED CT Subsets usually represent groups of concepts or objects (e.g. a specific clinical domain) which share specified characteristics.
Different types of Subsets are used to represent:
- Descriptions or concepts for particular realms or specialties
- Suitability of particular concepts for use in a particular context in a record
- Structure and ordering of hierarchies displaying concepts for user navigation

This records the streaming classification assigned to an Accident and Emergency Attendance, within an Accident and Emergency Department. Streaming is a system whereby PATIENTS are allocated to different flows according to their needs. These flows are individually staffed and continue to function whatever the pressures in other streams. Within any stream, at any one time, there may be PATIENTS with different A AND E INITIAL ASSESSMENT TRIAGE CATEGORIES. An Accident and Emergency Department will have either simple streaming or full streaming in place and will use the appropriate classification.

The NHS NUMBER, the primary identifier of a PERSON, is a unique identifier for a PATIENT within the NHS in England and Wales.
This will not vary by any ORGANISATION of which a PERSON is a PATIENT.
It is mandatory to record the NHS NUMBER. There are exceptions, such as Accident and Emergency care, sexual health and major incidents, as defined in existing national policies.
The NHS NUMBER is 10 numeric digits in length. The tenth digit is a check digit used to confirm its validity. The check digit is validated using the Modulus 11 algorithm and the use of this algorithm is mandatory. There are 5 steps in the validation of the check digit:
Step 1 Multiply each of the first nine digits by a weighting factor as follows:

Step 5 Check the remainder matches the check digit. If it does not, the NHS NUMBER is invalid.

- Codes for Executive Agency, Executive Agency Programme, Executive Agency Site and Executive Agency Programme Department do not easily fit into the coding frames as shown above and are therefore not included. This is due to their unusual structure in that there are more hierarchical 'tiers' than with other organisations.
Executive Agency and Executive Agency Programme are both considered Organisation level entities, although each Programme does have a relationship to an Executive Agency. Executive Agency codes are three characters long. Executive Agency Programme codes are six, and their first three characters are the same as the Executive Agency they are associated to.
Department codes of eight characters long can then be allocated underneath a Programme code (sharing the first six characters). Executive Agency Site codes of five characters long can be allocated under an Executive Agency code (and share the first three characters).
- A-9 indicates that characters A-Z and 0-9 are valid: except B, I, O, S, U and Z (to avoid ambiguity). This applies to all ORGANISATION CODES in the Coding Format Table above except Independent Sector Healthcare Providers (ISHP).
