A Two Year Neonatal Outcomes Assessment is a CARE CONTACT.
A Two Year Neonatal Outcomes Assessment is undertaken for all PATIENTS who were admitted for Neonatal Critical Care in a Neonatal Unit, transitional care setting or other non-standard critical care setting, at two years of age (adjusted for prematurity).
Responsibility for carrying out the Two Year Neonatal Outcomes Assessment lies with the Hospital Site where the PATIENT was born; or the Hospital Site where the PATIENT first received Neonatal Critical Care, if the Hospital Site of birth had no facilities to deliver Neonatal Critical Care (or the baby was not born in a Hospital Site).
