- an ORGANISATION, such as Commissioner Code for Ministry of Defence (MoD) Healthcare
- that a code value is not known
- that a code cannot be supplied (e.g. no referring GENERAL MEDICAL PRACTITIONER or GENERAL DENTAL PRACTITIONER).
