Background:
The NHS Data Model and Dictionary Service are currently working with the Department of Health and The NHS Information Centre for Health and Social Care to develop a number of Data Sets.
During the business analysis stage, it has been identified that these Data Sets cover a number of common areas which are either not currently in the NHS Data Model and Dictionary or amendments are required to make the current information suitable for use in these Data Sets.
This Change Request adds further common items and updates existing items as listed in the summary of changes below to support these new data sets.

Notes:The CIDS UNIQUE IDENTIFIER is used in conjunction with the ORGANISATION CODE (PROVIDER AT RECORD CREATION) to uniquely identify a record within a Community Information Data Set submission to the Secondary Uses Service.
When Net Change Community Information Data Set Submission Protocol is used, the CIDS UNIQUE IDENTIFIER and ORGANISATION CODE (PROVIDER AT RECORD CREATION) originally assigned to a record must be retained, otherwise duplicate Community Information Data Set records will be created in the Secondary Uses Service database.
When the CIDS UNIQUE IDENTIFIER is used in a Bulk Update Community Information Data Set Submission Protocol submission, it may be used to uniquely identify the record within that submission only, if systems are unable to maintain it across the life of the record in submissions to the Secondary Uses Service. However, it is strongly advised that users of the Bulk Replacement Community Information Data Set Submission Protocol do move towards the maintenance of a correctly generated CDS UNIQUE IDENTIFIER within the Community Information Data Set data. This will establish a migration path towards the use of the Net Change submission protocol, and will also then minimise the risk of creating duplicate Commissioning Data Set data in the Secondary Uses Service database.

Notes:ORGANISATION CODE (PROVIDER AT RECORD CREATION) is the ORGANISATION CODE (CODE OF PROVIDER) at the time that a CIDS UNIQUE IDENTIFIER for a record in a Community Information Data Set For Secondary Uses submission to the Secondary Uses Service was first created.
The Secondary Uses Service uses the ORGANISATION CODE (PROVIDER AT RECORD CREATION) and the CIDS UNIQUE IDENTIFIER to uniquely identify a Community Information Data Set record over its lifetime, when the Net Change Community Information Data Set Update Mechanism is used.
Therefore when using Net Change Submission Mechanism, it is imperative that, even if the ORGANISATION CODE (CODE OF PROVIDER) of the Health Care Provider changes (for example, there is a merger with another ORGANISATION), the ORGANISATION CODE (PROVIDER AT RECORD CREATION) carried in the record remains the same if the record is resubmitted to the Secondary Uses Service. If this is not done, a duplicate record will be created.
