Background:
This patch updates the NHS Data Model and Dictionary as follows:
- Updates the Data Element "Person Gender Code At Registration" to clarify the definition
- Makes the wording of all "Person Gender Data Elements" consistent.

Note that "0 Not Known" means that the sex of a PERSON has not been recorded. "9 Not Specified" means indeterminate, i.e. unable to be classified as either male or female.Notes:

PERSON GENDER CODE AT REGISTRATION is a PERSON's gender at registration.PERSON GENDER CODE AT REGISTRATION is a PERSON's gender at the time the birth was registered.
PERSON GENDER CODE AT REGISTRATION is the same as PERSON GENDER CODE where the PERSON GENDER TYPE is National Code 'Person Gender at Registration'.Where the PERSON has undergone a Gender Reassignment under the Gender Recognition Act, they will change their legal gender.
National Code 'Not Known' is not a permitted value for the National Workforce Data Set.
