- Website links updated
- HTML format corrected.

The current version of the Myocardial Ischaemia National Audit Project (MINAP) Data Set can be found at Royal College of Physicians website.
The last live version of this item is available in the June 2012 release of the NHS Data Model and Dictionary.

The Genitourinary Medicine Clinic Activity Data Set is used:

Description:
Public Health England require Sexual Health Services to generate and provide a data extract in accordance with the Genitourinary Medicine Clinic Activity Data Set. These SERVICES include:
- NHS providers of specialised Level 3 Genitourinary Medicine Services, where the primary function of the specialist clinical multidisciplinary team is concerned with the provision of screening, diagnosis and management of Sexually Transmitted Infections and related genital medical conditions.
- All Level 2 Sexual Health Services commissioned by the NHS who offer testing, diagnostic and/or treatment of Sexually Transmitted Infections.
It should be noted that General Practitioners with Special Interests (GPwSI) will only be included if they operate from a General Medical Practitioner Practice that has been commissioned to provide a Level 2 Sexual Health Service.
Time period: The extract must cover one calendar quarter.
Frequency: Reports must be run quarterly, 6 weeks after the end of the quarter.
Format: Data returned must be formatted into a single comma separated variable (csv) file. The data elements should be transmitted in the order specified in the Genitourinary Medicine Clinic Activity Data Set.

The Cancer Reform Strategy (2007) introduced new and changed commitments in terms of service standards for cancer PATIENTS that must be met. A Review of Cancer Waiting Times Standards was carried out by the Department of Health and published alongside Improving Outcomes: A Strategy for Cancer (2011).
Following this review it was confirmed in Improving Outcomes: A Strategy for Cancer that:
“overall, cancer waiting time standards should be retained. Shorter waiting times can help to ease patient anxiety and, at best, can lead to earlier diagnosis, quicker treatment, a lower risk of complications, an enhanced patient experience and improved cancer outcomes. - A maximum 62-day wait from referral from a cancer Screening Programme to First Definitive Treatment for all cancers
- A maximum 62-day wait from a CONSULTANTS decision to upgrade the urgency of a PATIENT they suspect to have cancer to First Definitive Treatment for all cancers
- A maximum 31-day wait for all subsequent treatments for new cases of primary and recurrent cancer where an Anti-Cancer Drug Regimen, surgery or Radiotherapy is the chosen CANCER TREATMENT MODALITY
- A maximum two week wait from referral for breast symptoms (where cancer is not initially suspected) to DATE FIRST SEEN.
Patient Pathway Scenarios:
The Patient Pathway Scenarios for the National Cancer Waiting Times Monitoring Data Set are to be used to manage the collection of data for all PATIENTS suspected of having, or diagnosed with cancer.
Cancer for the purpose of this data collection exercise is defined using the International Classification of Diseases (ICD) codes. Data for Patient Pathway Scenarios two to seven must be collected and transmitted as specified for all PATIENTS with a PRIMARY DIAGNOSIS within the range C00 to C97 or D05, or a secondary or metastatic disease linked to the original PRIMARY DIAGNOSIS (ICD) within this range (excluding categories relating to non-melanoma skin cancer).
A full list of the International Classification of Diseases (ICD) diagnosis codes the Cancer Waiting Times Database will accept is available at: Cancer Waiting Times.
When reporting patient records to the Cancer Waiting Times Database:
- The Trust first seeing a PATIENT in a particular month or quarter is responsible for ensuring that the mandated and required data fields, up to DATE FIRST SEEN, are complete on the database by the national deadline.
- Data to be complete and validated 25 working days after the REPORTING PERIOD END DATE, either month or quarter
- Specified dates are available at: Cancer Waiting Times.
Transmission:
- Information can be entered either manually through the Cancer Waiting Times Record screen (as an individual record) or via the available batch upload function.
- The specification for the csv upload file is detailed in the 'National Cancer Waiting Times User Manual' available at: Cancer Waiting Times.
- Further information relating to the data items required for the seven scenarios can be found at Patient Pathway Scenarios.
- The ability to transmit the data to the Cancer Waiting Times Database in XML format will be introduced from 1 October 2016 with the current csv upload function being discontinued from 1 April 2017. Data for submission will be formatted into an XML file as per Technology Reference Data Update Distribution (TRUD) at: NHS Data Model and Dictionary: DD XML Schemas.

The National Direct Access Audiology Waiting Times Data Set collects performance information on a monthly basis on the Referral To Treatment pathways of PATIENTS who are receiving NHS funded audiology treatment in England, who are not already included in the Referral To Treatment Data Set data collection. This includes:
- both analogue and digital hearing aid fittings
- services provided directly by NHS Healthcare Providers and also NHS funded PATIENTS treated via the Independent Sector and third sector providers (collected directly or via Primary Care Trusts)
- both new and existing PATIENTS
- any other PATIENTS attending Audiology services directly
For the purposes of the National Direct Access Audiology Waiting Times Data Set, "Direct Access" means PATIENTS who are not referred via Ear, Nose and Throat (ENT) specialist or other hospital CONSULTANT. Any pathways that are subject to the 18 week waiting time target for Referral to Treatment are out of scope. For this reason PATIENTS on Ear, Nose and Throat pathways (or pathways from other specialties) are excluded from this central return data set - information on these PATIENTS is available via the Referral To Treatment Data Set data collection.
The National Direct Access Audiology Waiting Times Data Set is in two parts, as follows:
Part 1 - Treated Patients should be completed for PATIENTS who have had an ACTIVITY which ends their REFERRAL TO TREATMENT PERIOD (such as first definitive treatment, or a decision not to treat), within the REPORTING PERIOD.
Part 2 - Untreated Patients should be completed for PATIENTS without a REFERRAL TO TREATMENT PERIOD END DATE within the REPORTING PERIOD.

Where the Allied Health Professional works in a Community Health Service, the Department of Health requires their Allied Health Professional Referral To Treatment Measurement activity to be reported in the Community Information Data Set.
Where the Allied Health Professional activity took place at an Out-Patient Clinic, the Allied Health Professional Referral To Treatment Measurement activity must be reported in the CDS V6-2 Type 020 - Outpatient Commissioning Data Set. In addition to this, where an Allied Health Professional sees a PATIENT on a WARD but the ACTIVITY is not related to the Hospital Provider Spell the PATIENT is being treated under, this should be regarded as replacing an Out-Patient Appointment Non-Consultant, and a CDS V6-2 Type 020 - Outpatient Commissioning Data Set record should flow. ACTIVITY LOCATION TYPE CODE may be submitted to allow identification of this Allied Health Professional ACTIVITY. For example, if a Podiatrist were asked to see a patient who was currently admitted for a condition where the agreed care pathway did not include Podiatry services, then an Out-Patient Appointment Non-Consultant should be recorded, with the ACTIVITY LOCATION TYPE CODE of E02 'WARD', and the relevant Referral To Treatment data items also completed.

The NHS Wales Informatics Service was established on 1 April, 2010, as part of the healthcare reform programme. It brings together the strategic development of Information Communications Technology (ICT), the delivery of operational ICT services and information management.
The new Organisation has a national remit to support the transformation of NHS Wales and make better use of scarce skills and resources. It was formed by merging Informing Healthcare, Health Solutions Wales, the Business Services Centre IM&T element, the Corporate Health Information Programme and the Primary Care Informatics Programme.

SITE CODE (OF TREATMENT) is the same as attribute ORGANISATION SITE CODE.
SITE CODE (OF TREATMENT) is the ORGANISATION SITE CODE of the Organisation where the PATIENT was treated, i.e. it should enable the treating Organisation to be identified.
This identifies the Organisation Site within the Organisation on which the PATIENT was treated, since facilities may vary on different hospital sites.
The code recorded should always be the national code; if the treatment is sub-commissioned to another NHS Health Care Provider or an independent UK provider, the SITE CODE (OF TREATMENT) used should be the ORGANISATION SITE CODE of the Health Care Provider actually carrying out the work.
Where treatment is sub-commissioned to an overseas provider the Organisation Data Service Default Code 89997 'Non-UK Provider where no ORGANISATION SITE CODE has been issued' is applicable.
Each Organisation has a unique ORGANISATION CODE. However, where an Organisation has more than one site from which it provides SERVICES, then each site is uniquely identified. These sites are Organisation Sites and are uniquely identified by ORGANISATION SITE CODE. The ORGANISATION SITE CODE contains the first 3 digits of the ORGANISATION CODE with the last two digits being the site identifier.

For out-patients, ACTIVITY may take place outside the hospital, such as in the PATIENT'S home; in such cases, raising a site code is impractical. Therefore, code R9998 'Not a hospital site (for use on Out-Patient CDS)' would be used in these circumstances. Therefore, code R9998 'Not a hospital site' would be used in these circumstances.
Note: LOCATION CLASS is used in the Commissioning Data Set (CDS) message to indicate the classification of the physical LOCATION within which the ACTIVITY occurred.
Use in the Future Outpatient CDS:
If the INTENDED SITE CODE (OF TREATMENT) is not known, this data element should be omitted.
