- HTML format corrected.

Following the issue of DSCN 07/2004 'Data Standards: Meta Model', the NHS Data Model and Dictionary was changed to reflect and be based upon a more generic logical data model which better supports the strategic way forward. This NHS Data Model and Dictionary was published as the NHS Data Model and Dictionary Version 3.
There was extensive quality assurance of Version 3 including involvement of the Data Definition Group. All Version 3 contents were cross mapped and checked to ensure consistency with the Version 2 contents but the underlying structure was genericised. No changes were made to NHS Business Definitions. Version 3 superseded Version 2 on 1st May 2005.

Summary of main changes and enhancements incorporated within Version 3
- Introduction of the capability of holding Retired Items
- Introduction of NHS Business Definitions, which allow specific business areas to be defined in a more flexible manner
- New ‘All Items Index’ which lists all the contents held within the NHS Data Model and Dictionary
- Reduction in the number of data model diagrams making them more comprehensible
- Introduction of Domains, which are conceptual logical modelling objects which identify the logical format, length and value set attributable to one or more attribute(s)
- 'Where Used' list expanded to include a description of usage column
- Creation of a separate Meta Model area within the publication
The introduction of Version 3 had no impact on current information standards or system suppliers.
About the Generic Model
Version 3 introduced a new Generic Model which is 'PERSON based' rather than 'ORGANISATION based'. The model has been developed around generic 'CARE ACTIVITIES' for a PATIENT, with each event transaction being recorded.
The Generic Model supports all the messages, data sets and central returns which the model supported but the underlying structure has been genericised. This involved grouping many of the old classes into generic classes like ACTIVITY GROUP. However, none of the detail was lost and most of the old class definitions can be found under 'NHS Business Definitions'. The number of diagrams has been greatly reduced and they now describe coherent areas of the model which is a far more useful approach for data modellers.
The model is aimed at enabling all 'CARE ACTIVITIES' related to the same condition for the same PATIENT to be recorded and linked across ORGANISATIONS. The different states of the same 'CARE ACTIVITY' are recorded as event transactions e.g. requested, intended, scheduled, provided, cancelled, etc. These different state events also drive the scheduling and capacity planning of resources (people, equipment, facilities, etc.) to deliver the care.
Although this may seem complicated, it is necessary both to form a coherent logical model and to relate physical information such as that which flows on the Data Sets to the logical model. Every physical item should be represented logically in the NHS Data Model and Dictionary. However, the scope of the logical model is greater than the physical information it holds and therefore not all logical information has a physical existence.

The Children's Global Assessment Scale (CGAS) is a type of ASSESSMENT TOOL.This item has been retired from the NHS Data Model and Dictionary.
It is a global rating of functioning aimed at children and young people aged 6-17 years. The child or young person is assessed against different levels of functioning from 0 "Needs constant supervision" to 100 "Superior functioning in all areas".The last live version of this item is available in the March 2018 release of the NHS Data Model and Dictionary.

Population Validation
All Data Elements are subject to length validation. Some Data Elements are also subject to format and content validation against a list of permitted values defined in the NHS Data Model and Dictionary. The value lists are held on the Attribute which the Data Element is based on, plus default codes which are held on the Data Element itself.

The Family Assessment Device (General Functioning Subscale) (FAD) is a type of ASSESSMENT TOOL.This item has been retired from the NHS Data Model and Dictionary.
The General Functioning Subscale of the Family Assessment Device is a 12-item questionnaire which constitutes part of the larger Family Assessment Device. The General Functioning Subscale has been widely used as a brief method of assessing overall family functioning. Each item is rated from 1 (Strongly Agree) to 4 (Strongly Disagree) with negatively worded items being reverse scored and missing values rated as 99. A Learning Disability (formerly known as a Mental Handicap and referred to as an Intellectual Disability) is a type of DISABILITY.
A Learning Disability usually has a significant impact on a PERSON's life. A PERSON with a Learning Disability finds it harder than others to learn, understand and communicate.
People with Profound and Multiple Learning Disabilities need full-time help with every aspect of their lives, including eating, drinking, washing, dressing and toileting etc.
Learning Disability includes the presence of a significantly reduced ability to understand new or complex information, to learn new skills (impaired intelligence), with
- a reduced ability to cope independently (impaired social functioning)
- which started before adulthood, with a lasting effect on development.

Although, from a social care perspective, an Intelligence Quotient (IQ) of seventy or less is not sufficient reason for deciding if an individual should be provided with additional health and social care support, the following definition is applied within the Mental Health Services Data Set in the specific circumstance where data providers do not have an explicit data item to capture whether a PATIENT has a Learning Disability.
Someone is considered to have a Learning Disability when they function at a level of intellectual ability which is significantly lower than their chronological age. This is usually considered to be equivalent to having an IQ of seventy or less:
- Mild Learning Disability (roughly equivalent to an IQ of fifty to seventy) is comparable to the educational term 'Moderate Learning Difficulty'. It is usually caused by a combination of restricted learning and social opportunities plus a high rate of low to average intellectual ability and Learning Disability in close relatives.
- Moderate-to-profound Learning Disability (roughly equivalent to an IQ below fifty) is comparable to the educational term 'Severe Learning Difficulty'. It usually has a specific biological cause.
Note: A Learning Difficulty is a type of Special Education Need which affects areas of learning, such as reading, writing, spelling, mathematics etc.

The Parenting Daily Hassles (PDH) is a type of ASSESSMENT TOOL.This item has been retired from the NHS Data Model and Dictionary.
The aim of Parenting Daily Hassles is to assess the frequency and intensity/impact of 20 experiences that can be a 'hassle' to parents.The last live version of this item is available in the March 2018 release of the NHS Data Model and Dictionary.
The caregiver is asked to score each of the potential 'hassles' in two different ways for frequency and intensity. The frequency of each type of happening provides an 'objective' marker of how often it occurs. The time frame for this scale can be varied according to the focus of the assessment. For example, if a family is thought to have been under particular pressure in the last 2 months, the parent can be asked to consider how matters have been during that period. However, if it is intended to assess progress, the same time frame should be used on each occasion. Periods of less than one month are probably too short to give a useful picture.

The Parenting Stress Index is designed for the early identification of parenting and family characteristics that fail to promote normal development and functioning in children, children with behavioural and emotional problems, and parents who are at risk of dysfunctional parenting.The last live version of this item is available in the March 2018 release of the NHS Data Model and Dictionary.
The Parenting Stress Index consists of 120 items, and yields a Total Stress Score, plus Scale Scores for both parent and child, which pinpoint sources of stress within the family.Access to this version can be obtained by emailing information.standards@nhs.net with "NHS Data Model and Dictionary - Archive Request" in the email subject line.

For PATIENTS treated in 'neonatal facilities', that is, in WARDS with a CRITICAL CARE UNIT FUNCTION of 13, 14 or 15, critical care PATIENTS include:
a) All PATIENTS on a WARD with a CRITICAL CARE UNIT FUNCTION Neonatal Intensive Care Unit regardless of care being delivered.
or
b) All PATIENTS (excluding Mothers) on a WARD with a CRITICAL CARE UNIT FUNCTION Facility for Babies on a Neonatal Transitional Care Ward or Facility for Babies on a Maternity Ward to whom one or more CRITICAL CARE ACTIVITIES with codes 01 to 02, 04 - 16 or 22 - 29 is delivered for a period greater than 4 hours.
For PATIENTS treated in 'adult facilities' or 'other facilities', that is, WARDS with a CRITICAL CARE UNIT FUNCTION of 01-03, 05-12, 90 or 91, the following conditions apply:
- A new CRITICAL CARE PERIOD starts when the PATIENT is admitted to a critical care location regardless of CRITICAL CARE LEVEL.
- Outreach activity, although part of critical care, should not be recorded as a CRITICAL CARE PERIOD.
- Resuscitation conducted outside designated critical care locations, e.g. as part of conventional care in operating theatres and Accident and Emergency Departments, should not be recorded as a CRITICAL CARE PERIOD even though many aspects of the care given may satisfy the definitions for Level 2 or Level 3 CRITICAL CARE LEVELS.
- Repeated admissions to the same unit should be recorded as separate CRITICAL CARE PERIODS identified by different CRITICAL CARE START DATES. A transfer to a different critical care location within the same Hospital Provider Spell will initiate a new CRITICAL CARE PERIOD identified by the differing CRITICAL CARE UNIT FUNCTION.
- A change of Consultant Episode (Hospital Provider) or brief transfers for investigation and/or treatment do not end the CRITICAL CARE PERIOD.
- A CRITICAL CARE PERIOD ends when the PATIENT is discharged from the critical care location, or dies, or the care that is being delivered in a non-standard location (see below) is CRITICAL CARE LEVEL National Code 00 Level 0 or 01 'Level 1'.
- Critical care locations are described by CRITICAL CARE UNIT FUNCTION and UNIT BED CONFIGURATION. Critical Care beds may include occasional non-standard locations using a ward area or operating department when conventional critical care beds are not available. Non standard locations may only be recorded if the CRITICAL CARE LEVEL is National Code 02 'Level 2' or 03 'Level 3' and the delivery of care is greater than four hours. For CRITICAL CARE PERIODS with CRITICAL CARE UNIT FUNCTIONS of either National Code 90 'non standard location using a ward area' or National Code 91 'non standard location using the operating department', care provided in these locations must exceed four hours, and must include CLINICAL INTERVENTIONS, monitoring and supervision normally associated with a critical care area i.e. intensive therapy unit or high dependency unit. Further the care provided must be continuously supervised by currently practising critical care doctors and NURSES who would normally work in critical care.
- The type of ORGAN SYSTEM SUPPORTED is recorded and the duration of each organ system support is calculated from the ACTIVITY PROPERTY EFFECTIVE DATE and the ACTIVITY PROPERTY END DATE.
- A CRITICAL CARE PERIOD does not include the following:
a. Surgical and anaesthetic intra-operative care
b. Post-operative care within an operating department except where level 2 or level 3 care are provided for more than 4 hours
c. Care delivered in a cardiac or coronary care unit
d. Imaging procedures
e. Endoscopy procedures
f. Care delivered in an Accident and Emergency Department
Commissioning Data Set Transmission
- The CRITICAL CARE PERIOD may overlap Episodes, i.e. the CRITICAL CARE START DATE may precede the start of the CONSULTANT/ MIDWIFE / NURSE Episode; similarly the CRITICAL CARE PERIOD may not have ended by the end of the Episode.
- The Data Elements CRITICAL CARE START DATE, CRITICAL CARE LOCAL IDENTIFIER and CRITICAL CARE UNIT FUNCTION must always be present.
- Where applicable, Support Days and Critical Care Level Days should only be entered when the CRITICAL CARE PERIOD is finished and the CRITICAL CARE DISCHARGE DATE is entered.
- The CRITICAL CARE DISCHARGE DATE must be on or before the DISCHARGE DATE (HOSPITAL PROVIDER SPELL).
