Contextual Overview

The National Neonatal Data Set consists of a defined list of data items that are extracted from electronic clinical records created by clinical staff on all admissions to Neonatal Critical Care Units in England.

The National Neonatal Data Set is in two parts:

• The National Neonatal Data Set - Episodic and Daily Care covers the period of time a baby is cared for in Neonatal Critical Care, Transitional Care, or other non-standard critical care settings
• The National Neonatal Data Set - Two Year Neonatal Outcomes Assessment, carries data relating to a Two Year Neonatal Outcomes Assessment carried out on the same child approximately two years after their treatment.  The Two Year Neonatal Outcomes Assessment may be carried out by the same Organisation who was responsible for the neonatal CRITICAL CARE PERIOD, or by a different Organisation.

The two neonatal data sets comprise data items relating to PATIENT demographics, CLINICAL INTERVENTIONS, outcomes, and PATIENT DIAGNOSES. Each data item is mapped where possible to existing ISB / SCCI / DCB Information Standards and Collections (including Extractions) (such as the Neonatal Critical Care Minimum Data Set and Maternity Services Data Set Version 1) as well as to SNOMED CT and ICD codes.

The aim of the National Neonatal Data Set is to extract data items from electronic clinical records, create a database of these items, and make this available as a national resource to serve a variety of needs, so avoiding duplicate data collections for different purposes, minimising the burden placed upon clinical teams, and promoting data quality and completeness.

Organisations involved in the collection may choose whether to allow identifiable or unidentifiable (anonymised) information to flow to the Neonatal Data Analysis Unit.  Where anonymised data is to flow, the appropriate 'withheld' patient and parents demographic structures should be used (i.e. those with no PERSON IDENTIFIERS, such as NHS NUMBER or PERSON BIRTH DATE). 

In addition, where anonymisation is required, the DATES AND TIMES of events carried throughout the data set (such as SAMPLE COLLECTION DATE AND TIME, PROCEDURE DATE AND TIME (ABDOMINAL X-RAY)) should be replaced with the specific relevant YEAR AND MONTH of the event and the NUMBER OF MINUTES (BIRTH TO EVENT).In addition, where anonymisation is required, the dates and times of events carried throughout the data set (such as SAMPLE COLLECTION DATE AND TIME, PROCEDURE DATE AND TIME (ABDOMINAL X-RAY)) should be replaced with the specific relevant year and month of the event and the NUMBER OF MINUTES (BIRTH TO EVENT).  The National Neonatal Data Set structure allows an either/or choice for these event items throughout the data set.

Data Collection

The National Neonatal Data Set consists of a defined list of data items that are extracted from electronic clinical records created by clinical staff relating to all neonatal critical care delivered in England.  The Neonatal Data Analysis Unit has established a database, the National Neonatal Research Database (NNRD) to hold data comprising the National Neonatal Data Set, as a national resource.

ACCOMMODATION STATUS RECORDED DATE is the DATE when the ACCOMMODATION STATUS CODE was recorded.ACCOMMODATION STATUS RECORDED DATE is the date when the ACCOMMODATION STATUS CODE was recorded.

For the Maternity Services Data Set ACTIVITY DATE (ANTENATAL APPOINTMENT) does not include the ACTIVITY DATE for a 'Pregnancy First Contact Date' or APPOINTMENT DATE (FORMAL ANTENATAL BOOKING).

ACTIVITY DATE (CRITICAL CARE) is the ACTIVITY DATE where the CARE ACTIVITY is during a CRITICAL CARE PERIOD.

ACTIVITY SERVICE REQUEST DATE (EMERGENCY CARE) is the DATE that a PATIENT was referred to another SERVICE during an Emergency Care Attendance.ACTIVITY SERVICE REQUEST DATE (EMERGENCY CARE) is the date that a PATIENT was referred to another SERVICE during an Emergency Care Attendance.

ACTIVITY SERVICE REQUEST TIME (EMERGENCY CARE) is the TIME that a PATIENT was referred to another SERVICE during an Emergency Care Attendance.ACTIVITY SERVICE REQUEST TIME (EMERGENCY CARE) is the time that a PATIENT was referred to another SERVICE during an Emergency Care Attendance.

AMERICAN JOINT COMMITTEE ON CANCER STAGE DATE is the DATE on which AMERICAN JOINT COMMITTEE ON CANCER STAGE was recorded during a Skin Cancer Care Spell.AMERICAN JOINT COMMITTEE ON CANCER STAGE DATE is the date on which AMERICAN JOINT COMMITTEE ON CANCER STAGE was recorded during a Skin Cancer Care Spell.

ANN ARBOR STAGE DATE is the DATE on which ANN ARBOR STAGE was recorded during a Haematological Cancer Care Spell.ANN ARBOR STAGE DATE is the date on which ANN ARBOR STAGE was recorded during a Haematological Cancer Care Spell.

Usage in the CDS:
The Outpatient and Future Outpatient CDS Types use the APPOINTMENT DATE as the "CDS ORIGINATING DATE" as a mandatory requirement of the CDS Exchange Protocol, see CDS ACTIVITY DATE.

For the Future Outpatient CDS where no APPOINTMENT DATE is available from the healthcare system, a default date value of 2999-12-31 may be applied. Care must be taken to generate the correct CDS Exchange Protocol when using this default value.

When submitting a Referral To Treatment Clock Stop Administrative Event via the CDS V6-2 Type 020 - Outpatient Commissioning Data Set, APPOINTMENT DATE is equivalent to the REFERRAL TO TREATMENT PERIOD END DATE carried in the record.

APPOINTMENT DATE (FORMAL ANTENATAL BOOKING) is the DATE of the Formal Antenatal Booking Appointment, i.e. the DATE of the first official Antenatal Booking Appointment at which the assessment for health and social care needs is carried out and Antenatal care arrangements for the Pregnancy Episode are made.APPOINTMENT DATE (FORMAL ANTENATAL BOOKING) is the date of the Formal Antenatal Booking Appointment, i.e. the date of the first official Antenatal Booking Appointment at which the assessment for health and social care needs is carried out and Antenatal care arrangements for the Pregnancy Episode are made.

APPRAISAL REVIEW PLANNED DATE (CONSULTANT JOB PLAN NEXT) is the planned review date for the next Appraisal Review of an EMPLOYEE PLAN where the EMPLOYEE PLAN TYPE is National Code 'Consultant Job Plan'.

ASSAULT DATE AND TIME is the DATE AND TIME the PATIENT was assaulted.ASSAULT DATE AND TIME is the date and time the PATIENT was assaulted.

ATTENDANCE DATE is the DATE of an attendance or contact, for example at a Consultant Clinic, Nurse Clinic, Accident and Emergency Department or by a Ward Attender.ATTENDANCE DATE is the date of an attendance or contact, for example at a Consultant Clinic, Nurse Clinic, Accident and Emergency Department or by a Ward Attender.

BARCELONA CLINIC LIVER CANCER STAGE DATE is the DATE on which BARCELONA CLINIC LIVER CANCER STAGE was recorded during a Liver Cancer Care Spell.BARCELONA CLINIC LIVER CANCER STAGE DATE is the date on which BARCELONA CLINIC LIVER CANCER STAGE was recorded during a Liver Cancer Care Spell.

BINET STAGE DATE is the DATE on which BINET STAGE was recorded during a Haematological Cancer Care Spell.BINET STAGE DATE is the date on which BINET STAGE was recorded during a Haematological Cancer Care Spell.

For the National Neonatal Data Set - Episodic and Daily Care, BLOOD SPOT CARD COMPLETION YEAR AND MONTH is submitted instead of BLOOD SPOT CARD COMPLETION DATE, where the data set record is anonymised.

Attribute:

CANCER DENTAL ASSESSMENT DATE is the DATE of the first dental assessment by a GENERAL DENTAL PRACTITIONER, which contributes to preparation for treatment, during a Head and Neck Cancer Care Spell.CANCER DENTAL ASSESSMENT DATE is the date of the first dental assessment by a GENERAL DENTAL PRACTITIONER, which contributes to preparation for treatment, during a Head and Neck Cancer Care Spell.

CANCER FASTER DIAGNOSIS PATHWAY END DATE is either the DATE when the PATIENT:CANCER FASTER DIAGNOSIS PATHWAY END DATE is either the date when the PATIENT:

• is informed of the CANCER FASTER DIAGNOSIS PATHWAY END REASON, where the National Code is either 'Diagnosis of cancer' or 'Ruling out of cancer'
• is excluded from the Cancer Faster Diagnosis Pathway, where the CANCER FASTER DIAGNOSIS PATHWAY END REASON is National Code is 'Excluded from the Cancer Faster Diagnosis Pathway'.

Note: where a Decision To Treat is made before the CANCER FASTER DIAGNOSIS PATHWAY END REASON is recorded, then the end of the Cancer Faster Diagnosis Pathway is the DECISION TO TREAT DATE.

A CANCER REFERRAL TO TREATMENT PERIOD START DATE will be one of the following:

• The REFERRAL REQUEST RECEIVED DATE of the SERVICE REQUEST:
  • to secondary care by a GENERAL MEDICAL PRACTITIONER or GENERAL DENTAL PRACTITIONER where the PRIORITY TYPE is National Code 'Two Week Wait' *
  • to secondary care where the PATIENT was subsequently upgraded onto a Cancer Pathway. The CONSULTANT UPGRADE DATE will also be recorded, as this is the DATE used to calculate the start of the two month (62 day) waiting time target for PATIENTS who have been upgraded to a Cancer Pathway
  • into secondary care when the PATIENT was referred urgently for 'breast symptoms', where the PRIORITY TYPE is National Code 'Two Week Wait' *
  • to an Assessment Clinic following the identification of an abnormality by an NHS Cancer Screening Service, where the PRIORITY TYPE is National Code 'Urgent' *
• The REFERRAL REQUEST RECEIVED DATE of the SERVICE REQUEST:
  • to secondary care by a GENERAL MEDICAL PRACTITIONER or GENERAL DENTAL PRACTITIONER where the PRIORITY TYPE is National Code 'Two Week Wait' *
  • to secondary care where the PATIENT was subsequently upgraded onto a Cancer Pathway. The CONSULTANT UPGRADE DATE will also be recorded, as this is the date used to calculate the start of the two month (62 day) waiting time target for PATIENTS who have been upgraded to a Cancer Pathway
  • into secondary care when the PATIENT was referred urgently for 'breast symptoms', where the PRIORITY TYPE is National Code 'Two Week Wait' *

• The ORIGINAL REFERRAL REQUEST RECEIVED DATE for the initial SERVICE REQUEST to secondary care by an NHS Cancer Screening Service, where the PRIORITY TYPE is National Code 'Routine', and where the PATIENT was subsequently upgraded onto a Cancer Pathway. The CONSULTANT UPGRADE DATE will also be recorded.

Notes:

• * The start of the Cancer Faster Diagnosis Pathway will be the CANCER REFERRAL TO TREATMENT PERIOD START DATE for the items annotated.
• For a SERVICE REQUEST received from the Choose and Book system, the referral is received when the PATIENT's Unique Booking Reference Number (UBRN) is used to book the first Out-Patient Appointment slot (i.e. converted). See REFERRAL REQUEST RECEIVED DATE.

CANCER SYMPTOMS FIRST NOTED DATE is the DATE when the symptoms were first noted related to the cancer diagnosis as agreed between the CONSULTANT and the PATIENT during a Cancer Care Spell.CANCER SYMPTOMS FIRST NOTED DATE is the date when the symptoms were first noted related to the cancer diagnosis as agreed between the CONSULTANT and the PATIENT during a Cancer Care Spell.

Note:

• Depending on the length of time this should normally include at least the month and year. The day should also be included if known. If the symptoms have been present for a long time then it may only be possible to record the year.
• In these circumstances the Format/Length will be:
  • DATE (including year, month and day): CCYY-MM-DD
  • YEAR AND MONTH: YYYY-MM
  • Year only: YYYY.
• In these circumstances the Format/Length will be:
  • Date (including year, month and day): CCYY-MM-DD
  • Year and month: YYYY-MM
  • Year only: YYYY.

A CANCER TREATMENT PERIOD START DATE will be either:

• the DECISION TO TREAT DATE
  •  the DATE that a PATIENT agrees a treatment plan for either first or subsequent treatments within a Cancer Care Plan.  An individual PATIENT may have multiple DECISION TO TREAT DATES or
• the DECISION TO TREAT DATE
  • the date that a PATIENT agrees a treatment plan for either first or subsequent treatments within a Cancer Care Plan.  An individual PATIENT may have multiple DECISION TO TREAT DATES or
• the EARLIEST CLINICALLY APPROPRIATE DATE
  • where there is no new DECISION TO TREAT DATE, but there has been a previously agreed and clinically appropriate period of delay. In this case the subsequent ACTIVITY may not be the final treatment, but could be the next APPOINTMENT which deals with the planning of subsequent treatments.

CARE CONTACT DATE (DIETITIAN INITIAL) is the DATE of the Initial Contact with a Dietitian.CARE CONTACT DATE (DIETITIAN INITIAL) is the date of the Initial Contact with a Dietitian.

CARE CONTACT DATE (SPEECH AND LANGUAGE THERAPIST INITIAL) is the DATE of the Initial Contact with a Speech and Language Therapist.CARE CONTACT DATE (SPEECH AND LANGUAGE THERAPIST INITIAL) is the date of the Initial Contact with a Speech and Language Therapist.

For the Mental Health Services Data Set, where the CARE PLAN has not been updated since its creation, the CARE PLAN LAST UPDATED DATE will be the same as CARE PLAN CREATION DATE.

For the Mental Health Services Data Set, where the CARE PLAN has not been updated since its creation, the CARE PLAN LAST UPDATED TIME will be the same as CARE PLAN CREATION TIME.

CARE PROGRAMME APPROACH REVIEW DATE is the DATE of the Care Programme Approach Review.CARE PROGRAMME APPROACH REVIEW DATE is the date of the Care Programme Approach Review.

The CDS ACTIVITY DATE has an associated "CDS Originating Date" specifically identified for each CDS Type as follows:

Usage: 
The CDS ACTIVITY DATE is validated by the Secondary Uses Service and Commissioning Data Set Interchanges are rejected if the date is not present, invalid or not compatible with the Commissioning Data Set Submission Protocol controls being used.

In particular, when using the Commissioning Data Set Bulk Replacement Update Mechanism, the CDS ACTIVITY DATE and its "CDS Originating Date" are used by the Secondary Uses Service to validate that the CDS Type date applicability falls within the CDS REPORT PERIOD START DATE and the CDS REPORT PERIOD END DATE.

Usage: 
CDS APPLICABLE DATE is mandatory when used with the Commissioning Data Set Net Change Update Mechanism. It is not required when the Commissioning Data Set Bulk Replacement Update Mechanism is used. See the Commissioning Data Set Submission Protocol.

The CDS APPLICABLE DATE (and the CDS APPLICABLE TIME if supplied) is stored in the Secondary Uses Service database and in the event of multiple submissions of the same uniquely identified Commissioning data (even in separate interchanges).

The Secondary Uses Service database update process is then able to use this date and time to ensure correct updating of the Commissioning data in the correct relative date/time sequence.

Usage: 
CDS APPLICABLE TIME is mandatory when used with the Commissioning Data Set Net Change Update Mechanism. It is not required when the CDS Bulk Replacement Update Mechanism is used. See the Commissioning Data Set Submission Protocol.

The CDS APPLICABLE TIME (and CDS APPLICABLE DATE if supplied) is stored in the Secondary Uses Service database and in the event of multiple submissions of the same uniquely identified Commissioning data (even in separate interchanges), the Secondary Uses Service database update process is then able to use the date and time to ensure correct updating of the Commissioning data in the correct relative date/time sequence.

Usage: 
CDS CENSUS DATE is required when submitting the Long Term and/or Detained Psychiatric Census CDS Type.

Usage: 
CDS EXTRACT DATE is mandatory when used with the Commissioning Data Set Bulk Replacement Update Mechanism. It is not required when the Commissioning Data Set Net Change Update Mechanism is used, see the Commissioning Data Set Submission Protocol.

The CDS EXTRACT DATE (and CDS EXTRACT TIME if supplied) is used by the Secondary Uses Service to detect duplicate Interchanges of a similarly defined Bulk Update submission of Commissioning Data Sets.

The Secondary Uses Service processes and stores the date and time information to ensure correct updating of the Commissioning Data Set data in the correct relative date/time sequence.

Usage: 
CDS EXTRACT TIME is mandatory when using the Commissioning Data Set Bulk Replacement Update Mechanism and is used to ensure that submissions are processed in the correct relative sequence. See the Commissioning Data Set Submission Protocol.

The sender of Commissioning Data Set data should determine the most useful point of the system's processes to generate this time value to provide a useful reference/audit control point.

Usage: 
CDS INTERCHANGE DATE OF PREPARATION is a mandatory data element when submitting Commissioning data.

CDS XML Schema Interchanges:
All CDS XML Schema interchanges submitted must contain a CDS INTERCHANGE DATE OF PREPARATION.

Usage: 
CDS INTERCHANGE TIME OF PREPARATION is a mandatory data element when submitting Commissioning Data Set Interchanges.

CDS XML Schema Interchanges:
All CDS XML Schema interchanges submitted to the service must contain a CDS INTERCHANGE TIME OF PREPARATION.

Usage: 
CDS REPORT PERIOD END DATE is a mandatory data item when the Commissioning Data Set Bulk Replacement Update Mechanism is used. It is not required when the Commissioning Data Set Net Change Update Mechanism is used.
The CDS REPORT PERIOD END DATE must be a valid date and must not be before the CDS REPORT PERIOD START DATE.

Usage: 
CDS REPORT PERIOD START DATE  is a mandatory data item when the Commissioning Data Set Bulk Replacement Update Mechanism is used. It is not required when the Commissioning Data Set Net Change Update Mechanism is used.
The CDS REPORT PERIOD START DATE must be a valid date and cannot be after the CDS REPORT PERIOD END DATE.

CHANG STAGING SYSTEM STAGE DATE is the DATE on which CHANG STAGING SYSTEM STAGE was recorded during a Central Nervous System Cancer Care Spell.CHANG STAGING SYSTEM STAGE DATE is the date on which CHANG STAGING SYSTEM STAGE was recorded during a Central Nervous System Cancer Care Spell.

CHILD PROTECTION PLAN END DATE is the DATE on which a Child or Young Person is removed from a Child Protection Plan.CHILD PROTECTION PLAN END DATE is the date on which a Child or Young Person is removed from a Child Protection Plan.

CHILD PROTECTION PLAN START DATE is the DATE on which a Child or Young Person is placed on a Child Protection Plan.CHILD PROTECTION PLAN START DATE is the date on which a Child or Young Person is placed on a Child Protection Plan.

CLINICAL STAGE DATE (PANCREATIC CANCER) is the DATE on which CLINICAL STAGE (PANCREATIC CANCER) was recorded during a Upper Gastrointestinal Cancer Care Spell.CLINICAL STAGE DATE (PANCREATIC CANCER) is the date on which CLINICAL STAGE (PANCREATIC CANCER) was recorded during a Upper Gastrointestinal Cancer Care Spell.

CLINICAL STATUS ASSESSMENT DATE (CANCER) is the DATE on which a clinical status assessment was performed during a Head and Neck Cancer Care Spell.CLINICAL STATUS ASSESSMENT DATE (CANCER) is the date on which a clinical status assessment was performed during a Head and Neck Cancer Care Spell.

For the Cancer Outcomes and Services Data Set, if the PATIENT enters into more than one CLINICAL TRIAL, CLINICAL TRIAL DECISION DATE should be recorded for each CLINICAL TRIAL.

CRITICAL CARE DISCHARGE DATE may be the:

• date the PATIENT is discharged from the critical care unit
• date the PATIENT died or
• date of declaration of brainstem death.

CRITICAL CARE DISCHARGE DATE is used by the Secondary Uses Service to derive the Healthcare Resource Group 4. Failure to correctly populate this data element is likely to result in an incorrect Healthcare Resource Group, usually associated with lower levels of healthcare resource.
