This patch updates the NHS Data Model and Dictionary in preparation for the new release and includes:
- What's New amended to include Change Requests incorporated since the last version of the NHS Data Model and Dictionary was published

- HTML format corrected.

Autistic Spectrum Disorder is a DISABILITY.
Autistic Spectrum Disorder is defined as a lifelong condition that affects how a PERSON communicates with, and relates to, other people. Autistic Spectrum Disorder also affects how a PERSON makes sense of the world around them.
The three main areas of difficulty, which all people with Autism share, are known as the 'triad of impairments'. They are
- difficulty with social communication (e.g. problems using and understanding verbal and non-verbal language, such as gestures, facial expressions and tone of voice)
- difficulty with social interaction (e.g. problems in recognising and understanding other people's feelings and managing their own feelings)
- difficulty with social imagination (e.g. problems in understanding and predicting other people's intentions and behaviour and imagining situations outside their own routine)

Breslow Thickness is a CLINICAL INVESTIGATION RESULT ITEM.
Breslow Thickness (also known as Breslow Depth) is a measure of how deeply a melanoma Tumour has grown into the skin.
The Tumour thickness (depth) is usually measured from the top of the Tumour to the deepest Tumour CELLS. If the Tumour is ulcerated (the skin is broken), it is measured from the base of the ulcer to the deepest Tumour CELLS.
Breslow Thickness is used to help determine the stage of cancer. Thicker Tumours are linked with lower survival rates.

The Care Quality Commission is an ORGANISATION.
The Care Quality Commission is the independent regulator of all health and adult social care services in England, whether provided by the NHS, Local Authorities, private companies or voluntary ORGANISATIONS. It also protects the rights of people detained under the Mental Health Act.
The Care Quality Commission makes sure that essential common quality standards are being met where care is provided and works towards the improvement of care services. It promotes the rights and interests of people who use services and has a wide range of enforcement powers to take action on their behalf if services are unacceptably poor.
The Care Quality Commission's work brings together independent regulation of health, mental health and adult social care. Before 1 April 2009, this work was carried out by the Healthcare Commission, the Mental Health Act Commission and the Commission for Social Care Inspection. These ORGANISATIONS no longer exist.
The Care Quality Commission’s main activities are:
- Registration of health and social care providers to ensure they are meeting essential common quality standards;
- Monitoring and inspection of all health and adult social care;
- Using its enforcement powers, such as fines and public warnings or closures, if standards are not being met;
- Improving health and social care services by undertaking regular reviews of how well those who arrange and provide services locally are performing and special reviews on particular care services, pathways of care or themes where there are particular concerns about quality;
- Reporting the outcomes of its work so that people who use services have information about the quality of their local health and adult social care services. It helps those who arrange and provide services to see where improvement is needed and learn from each other about what works best.

Child and Adolescent Mental Health Needs Based Groupings are groupings that categorise the need for advice or help of children, young people and/or families referred to a SERVICE.
A Child and Adolescent Mental Health Needs Based Grouping should be assigned via a process of shared decision making between the SERVICE provider and the PATIENT or Patient Proxy. The decision, undertaken by a CARE PROFESSIONAL, may be helped by using an algorithm that was created to develop the groupings.

A Learning Disability (formerly known as a Mental Handicap and referred to as an Intellectual Disability) is a type of DISABILITY.
A Learning Disability usually has a significant impact on a PERSON's life. A PERSON with a Learning Disability finds it harder than others to learn, understand and communicate.
People with Profound and Multiple Learning Disabilities need full-time help with every aspect of their lives, including eating, drinking, washing, dressing and toileting etc.
Learning Disability includes the presence of a significantly reduced ability to understand new or complex information, to learn new skills (impaired intelligence), with
- a reduced ability to cope independently (impaired social functioning)
- which started before adulthood, with a lasting effect on development.
Further information regarding people covered by definition can be found in the document 'Valuing People: A New Strategy for Learning Disability for the 21st Century'.
Although, from a social care perspective, an Intelligence Quotient (IQ) of seventy or less is not sufficient reason for deciding if an individual should be provided with additional health and social care support, the following definition is applied within the Mental Health Services Data Set in the specific circumstance where data providers do not have an explicit data item to capture whether a PATIENT has a Learning Disability.
Someone is considered to have a Learning Disability when they function at a level of intellectual ability which is significantly lower than their chronological age. This is usually considered to be equivalent to having an IQ of seventy or less:
- Mild Learning Disability (roughly equivalent to an IQ of fifty to seventy) is comparable to the educational term 'Moderate Learning Difficulty'. It is usually caused by a combination of restricted learning and social opportunities plus a high rate of low to average intellectual ability and Learning Disability in close relatives.
- Moderate-to-profound Learning Disability (roughly equivalent to an IQ below fifty) is comparable to the educational term 'Severe Learning Difficulty'. It usually has a specific biological cause.
Note: A Learning Difficulty is a type of Special Education Need which affects areas of learning, such as reading, writing, spelling, mathematics etc.

An Ultrasound Scan (also known as a Sonogram) is a procedure that uses high frequency sound waves to create an image of part of the inside of the body, such as the heart.
Ultrasound Scans can be used:
- to help doctors make a diagnosis or to assess the effects of a treatment
- to study blood flow and to detect any narrowing or blocking of blood vessels
- during a Maternity Episode to produce images of the baby in the womb.

For out-patients, ACTIVITY may take place outside the hospital, such as in the PATIENT'S home; in such cases, raising a site code is impractical. Therefore, code R9998 'Not a hospital site' would be used in these circumstances.

The ORGANISATION SITE CODE contains the first 3 digits of the ORGANISATION CODE with the last two digits being the site identifier.
