A Consultant Upgrade Date is the date of the Consultant Upgrade.

The Consultant Upgrade Date should only be recorded when the PRIORITY TYPE of the original SERVICE REQUEST was not National Code 'Two Week Wait'.

The Consultant Upgrade Date must be:

• on or before the DECISION TO TREAT DATE (if recorded) and 
• on or before the Multidisciplinary Team Discussion Date (Cancer) (if recorded).
