The Electronic Prescribing and Medicines Administration Data Sets have been incorporated early to allow users to see the changes, but please note that the implementation date is 13 January 2025.
Introduction
The Electronic Prescribing and Medicines Administration Data Sets are secondary use data sets which aim to re-purpose clinical and operational data concerning the prescribing and administration of medication in Health Care Providers in England who have a live Electronic Prescribing and Medication Administration system.
The utilisation of secondary care PATIENT-level data supports improvements to health outcomes, and the Electronic Prescribing and Medicines Administration Data Sets will be linked to other national data sets to provide enhanced intelligence to support:
- safety and effectiveness
- monitoring uptake of innovative medicine
- pharmacovigilance
- cost-effectiveness (pharmacoeconomics)
- compliance with best practice and policy directives
- antimicrobial stewardship
Data in scope of the Electronic Prescribing and Medicines Administration Data Sets is from the Primary Electronic Prescribing and Medicine Administration system. Therefore the data should not include activity where a separate system is used, for example for the delivery of certain cancer treatments (such as those covered by the Systemic Anti-Cancer Therapy Data Set), the prescribing and administration of specific medications such as Warfarin, and delivery of medications in specific care settings such as Intensive Care Units.
Care settings in scope of the Electronic Prescribing and Medicines Administration Data Sets are:
- Admitted Patient Setting - on a WARD during a Hospital Provider Spell
- Out-Patient Setting - in an Out-Patient Clinic or Ward Attendance
- Emergency Care Department setting
- Admitted Patient - on Discharge from the Hospital Provider Spell
- Admitted Patient - for Home Leave
- Admitted Patient - existing medication on admission
- Other Medication Administration Setting
Submission Information
Electronic Prescribing and Medicines Administration Data Sets data for submission must be formatted into XML files as per the Technology Reference Update Distribution (TRUD) page at: NHS Data Model and Dictionary XML Schemas.
The data sets should be extracted from the primary Electronic Prescribing and Medicine Administration system on a weekly basis, covering the previous seven days' ACTIVITY for both prescribing and medicines administration.
Data submissions are made to NHS England using the Message Exchange for Social Care and Health (MESH) portal.

An Emergency Care Attendance is a CARE CONTACT.
An Emergency Care Attendance is an individual visit by one PATIENT to an Emergency Care Department to receive treatment.
An Emergency Care Attendance may occur as an unscheduled presentation by the PATIENT themselves, but also as a result of a REFERRAL REQUEST from a GENERAL MEDICAL PRACTITIONER, a direct transfer from an Emergency Ambulance, or via a REFERRAL REQUEST from the NHS 111 Service.
An Emergency Care Attendance may be conducted face to face, or virtually by a CARE PROFESSIONAL from an Emergency Care Department who is qualified to deliver virtual care. In Emergency Care Data Set Version 4, the CONSULTATION MECHANISM (URGENT AND EMERGENCY CARE) is used to differentiate face to face attendances from those conducted virtually.
An Emergency Care Attendance may be either a first or follow up attendance. A follow-up attendance is any subsequent Emergency Care Attendance for the same condition, and this may be planned or unplanned, as defined by the URGENT AND EMERGENCY CARE ATTENDANCE CATEGORY (ECDS V4) / EMERGENCY CARE ATTENDANCE CATEGORY (CDS 6-2-3). All planned follow-up Emergency Care Attendances should follow a clinical decision to discharge the PATIENT to their normal place of residence, but with an ongoing duty of care to follow up. For Emergency Care Data Set Version 4, where a follow up Emergency Care Attendance is planned, this triggers the start of an Urgent and Emergency Care Extended Care Episode. The Urgent and Emergency Care Extended Care Episode starts at the Urgent and Emergency Care Activity End Timestamp of the Emergency Care Attendance within which the clinical decision that there should be an ongoing duty of care to follow up was taken.
If a PATIENT with an open Urgent and Emergency Care Extended Care Episode attends the Emergency Care Department outside of a scheduled attendance in the Urgent and Emergency Care Extended Care Episode (for example their condition deteriorates and they attend the Emergency Care Department), the unscheduled Emergency Care Attendance should also be recorded as being part of the open Urgent and Emergency Care Extended Care Episode.
The URGENT AND EMERGENCY CARE ACTIVITY IDENTIFIER which identifies the open Urgent and Emergency Care Extended Care Episode must be linked in the ELECTRONIC HEALTH RECORD system, and submitted in the Emergency Care Data Set Version 4 record relating to the Emergency Care Attendance, for all planned and unplanned Emergency Care Attendances within the Urgent and Emergency Care Extended Care Episode.
During an Emergency Care Attendance the PATIENT may temporarily leave the Emergency Care Department, for example for a Clinical Investigation, whilst still under the clinical responsibility of the Emergency Care Department. Such temporary absences from the Emergency Care Department do not interrupt the Emergency Care Attendance for that PATIENT.
During a Emergency Care Attendance, when the clinical decision is made that the PATIENT no longer requires ongoing care by the Emergency Care Department, this is recorded as the Urgent and Emergency Care Clinically Ready To Proceed Timestamp.During an Emergency Care Attendance, when the clinical decision is made that the PATIENT no longer requires ongoing care by the Emergency Care Department, this is recorded as the Urgent and Emergency Care Clinically Ready To Proceed Timestamp.
For a face to face Emergency Care Attendance (where the CONSULTATION MECHANISM (URGENT AND EMERGENCY CARE) is National Code ‘Face to face’), the Urgent and Emergency Care Activity End Timestamp (ECDS V4) / Emergency Care Departure Date and Emergency Care Departure Time (CDS v6-2-3) is the time that the PATIENT leaves the Emergency Care Department premises and their clinical care is no longer the responsibility of the Emergency Care Department.
For an Emergency Care Attendance undertaken virtually (where the CONSULTATION MECHANISM (URGENT AND EMERGENCY CARE) is National Codes ‘Telephone’, ‘Video Consultation’ or ‘Chat Room (Synchronous)’), the Urgent and Emergency Care Activity End Timestamp (ECDS V4) / Emergency Care Departure Date and Emergency Care Departure Time (CDS v6-2-3) is the time that all CARE ACTIVITIES undertaken by the responsible CARE PROFESSIONAL relating to the Emergency Care Attendance are completed.
An Emergency Care Attendance (face to face or virtual) outcome may include:
- The clinical care of the PATIENT is transferred to a different Health Care Provider and the PATIENT leaves the Emergency Care Department premises. This may be before the PATIENT has been formally assessed by a treating CARE PROFESSIONAL (termed ‘Streaming’), or after formal initial assessment (as recorded in the Urgent and Emergency Care Initial Assessment Timestamp), when it is a transfer of care based on that clinical assessment by the responsible CARE PROFESSIONAL in the Emergency Care Department
- The PATIENT leaves the Emergency Care Department following formal discharge, with no further clinical care planned as part of an Urgent and Emergency Care Extended Care Episode
- A decision to admit to a Hospital Provider Spell is taken by the responsible CARE PROFESSIONAL
- The PATIENT leaves the Emergency Care Department for a Same Day Emergency Care Attendance at the same Health Care Provider
- The PATIENT is assessed during the Emergency Care Attendance as requiring ongoing clinical care from the same Urgent and Emergency Care Service, which is to be delivered under a Urgent and Emergency Care Extended Care Episode.
If during the Emergency Care Attendance the PATIENT leaves the Emergency Care Department premises (if being seen face to face), or does not continue engagement with the assigned CARE PROFESSIONAL during virtual care, then the Emergency Care Attendance is completed. The Urgent and Emergency Care Activity End Timestamp (ECDS V4) / Emergency Care Departure Date and Emergency Care Departure Time (CDS v6-2-3) should record the time at which the CARE PROFESSIONAL responsible for the Emergency Care Attendance made the clinical decision to provide no further care. The URGENT AND EMERGENCY CARE DISCHARGE STATUS (SNOMED CT) should indicate that the PATIENT has left the Emergency Care Department/declined further virtual care, and that clinical care responsibility transfers back to their own usual CARE PROFESSIONAL (for example their GENERAL MEDICAL PRACTITIONER).
An Emergency Care Attendance should be recorded where a PATIENT who is admitted to a Hospital Provider Spell and Care Professional Admitted Care Episode at the same Health Care Provider, is taken to the Emergency Care Department for treatment because, for example, the required resources are not available on the WARD to which the PATIENT is admitted. The Hospital Provider Spell and Care Professional Admitted Care Episode are not interrupted by an Emergency Care Attendance of this nature. The Emergency Care Attendance record should identify the appropriate URGENT AND EMERGENCY CARE ATTENDANCE SOURCE (SNOMED CT) SNOMED CT concept for a PATIENT attending an Emergency Care Department from a WARD setting.
Health Care Providers should note, however, that before they commence such recording of Emergency Care Attendances for admitted PATIENTS, they should discuss this with their commissioners (e.g. Integrated Care Board). If the commissioner is not willing to fund the Emergency Care Attendance in addition to the Care Professional Admitted Care Episode, then the Emergency Care Data Set Version 4 record must be excluded from the National Tariff Payment System process. This is done by following the guidance on the NHS SERVICE AGREEMENT IDENTIFIER data element in the relevant Emergency Care Data Set Version 4 record.

- New National Codes for MAIN SPECIALTY CODE and TREATMENT FUNCTION CODE were introduced from 2 April 2020 as part of the update to the DCB0028: Treatment Function and Main Specialty Standard. Submission of these codes for the Commissioning Data Sets is only possible where the healthcare provider has updated their CDS-XML schema version to CDS-XML version 6-2-0. Users of the original CDS-XML schema version 6-2 will be unable to submit the new codes introduced in the release of DCB0028: Treatment Function and Main Specialty Standard in April 2020 or the addendum to DCB0028 released in January 2021 to add a new TREATMENT FUNCTION CODE to represent Post-COVID-19 Syndrome Services.

The National Interim Clinical Imaging Procedure Code Set (NICIP Code Set) is a comprehensive national standard set of codes and descriptions for imaging procedures and is maintained by NHS England. It is intended for use in all Imaging Department information systems.
The NICIP Code Set was approved by the Information Standards Board for Health and Social Care (ISB) and is mandated for all in-scope use cases. Further detail about the initial information standard and subsequent amendments can be found on the Information Standards Board for Health and Social Care website at: ISB 0148 "Interim Clinical Imaging Procedure Codes".
The NICIP Code Set is released biannually. The release dates are the 1st of April and the 1st of October each year.
All versions of the NICIP Code Set, both with and without SNOMED CT maps, are only available from the Technology Reference Update Distribution (TRUD).
Clinicians and system managers working with the Picture Archiving and Communication Systems (PACS) and Radiology Information Systems (RIS) can make requests for additions to the NICIP Code Set. Introduction
The Radiotherapy Data Set (RTDS) allows for the routine collection of clinically and managerially relevant ACTIVITY data from Radiotherapy centres to provide operational data to enable bench marking and to support enhancing clinical practice, in addition to commissioning or monitoring Radiotherapy Services in an evidence-based manner.
Radiotherapy is a major modality in the treatment of cancer and also represents a significant sector within the NHS, in terms of both workforce and capital investment.
All NHS funded facilities in England providing Radiotherapy Services are required to return data to Public Health England (PHE) for all ACTIVITY undertaken on External Beam Radiotherapy (Teletherapy) and Brachytherapy MACHINES, or with radioisotopes not contained within an External Beam Radiotherapy (Teletherapy) or Brachytherapy MACHINE.
The Radiotherapy Data Set accompanies the Out-Patient Commissioning Data Set for PATIENTS attending for Radiotherapy.
Where admitted PATIENTS attend for Radiotherapy, a Radiotherapy Attendance record should be submitted to Public Health England along with the Out-Patient Commissioning Data Set for the In-Patient Attendance.
Data Submission
The Radiotherapy Data Set should be submitted to Public Health England by the 20th working day of each month.The Radiotherapy Data Set should be submitted to the National Disease Registration Service (NDRS) by the 20th working day of each month. The extracts should include all Radiotherapy Attendance records for the previous calendar month.
Data should be submitted using the National Cancer Registration and Analysis Service (NCRAS) upload portal at: NCRAS Login.

SNOMED CT® is the clinical terminology approved as an Information Standard.
Requirements for utilising SNOMED CT® are stated within the National Information Board document "A Framework for Action" and SNOMED CT® can be downloaded from Technology Reference Update Distribution (TRUD).
SNOMED CT® provides the clinical language that facilitates electronic communication between healthcare professionals in clear and unambiguous terms, and can be used to code, retrieve and analyse clinical data.
SNOMED CT® is comprehensive and provides clinical terms for all healthcare professions. Applications often use subsets of SNOMED CT®, known as SNOMED CT Refsets, that have been developed to support specific requirements. The NHS Data Model and Dictionary references SNOMED CT Refsets to support data reporting for specific data items.
SNOMED CT® has been distributed in Release Format 2 (RF2) since 1 April 2018. SNOMED CT® contains SNOMED CT Refsets referenced by SNOMED CT Refset ID, which are listed in the SNOMED CT Browser.
SNOMED CT® is managed and maintained internationally by SNOMED International and in the UK by NHS England.
National and International arrangements have been established to ensure there is adequate and relevant governance of SNOMED CT®, to ensure it meets the needs of healthcare in the respective jurisdictions.

Introduction
The Systemic Anti-Cancer Therapy Data Set collects clinical management information on PATIENTS undergoing Systemic Anti-Cancer Therapy in (or funded by) the NHS in England.
In the clinical setting its primary use is in prescribing and administering Systemic Anti-Cancer Therapy. The standard also specifies secondary uses information about Systemic Anti-Cancer Therapy which is required to assist in achieving, supporting and monitoring the NHS Operating Framework, specialised commissioning and related policy.
Details of Standard
The Systemic Anti-Cancer Therapy Data Set relates to all cancer PATIENTS, both adult and paediatric, in both admitted PATIENT care and outpatient and community settings, who are receiving Systemic Anti-Cancer Therapy for all solid Tumour and haematological malignancies, including those treated in CLINICAL TRIALS.
The Systemic Anti-Cancer Therapy Data Set covers the period from the Start Date of the Systemic Anti-Cancer Therapy Drug Regimen to the Start Date of the last Systemic Anti-Cancer Therapy Drug Cycle.
Note: PATIENTS may end their treatment with an oral or other component taken at home. Although the dispensing of this will be recorded in the final Systemic Anti-Cancer Therapy Drug Cycle, it is not possible to confirm that the PATIENT has taken the medication.
Data Set Structure and Transmission
Systemic Anti-Cancer Therapy is given over a prolonged period of time, often months or years, comprising repeating and sequential elements. The PATIENT may attend two or more Health Care Providers during the course of treatment.
In order to track the PATIENT during treatment, the data set must be capable of linking all the elements of care in a consistent and ordered way. In order to achieve this, the Systemic Anti-Cancer Therapy Data Set has a branching structure which links the initial data fields, which will remain constant during the treatment, with detail of each Systemic Anti-Cancer Therapy Drug Regimen, Systemic Anti-Cancer Therapy Drug Cycle and Systemic Anti-Cancer Therapy Drug Administration. At the completion or cessation of a Systemic Anti-Cancer Therapy Drug Regimen, the outcome section must link back to all previous fields. It must be possible to reconstitute details of each PATIENT’s sequential management from the serial downloads received.
All NHS Trusts and NHS Foundation Trusts providing Systemic Anti-Cancer Therapy Services are required to submit monthly data downloads to an agreed timetable, two months in arrears. These data must represent all treatment activity in the month period, including Systemic Anti-Cancer Therapy Drug Regimens started and completed or ceased in the REPORTING PERIOD.
The data repository is hosted by the National Cancer Registration and Analysis Service (NCRAS) and the data is held under their section 251 of the National Health Service Act 2006.

ORGANISATION CODE (CODE OF COMMISSIONER) is the ORGANISATION CODE of the ORGANISATION commissioning health care.
For Commissioning Data Sets, the ORGANISATION CODE (CODE OF COMMISSIONER) should always be the ORGANISATION CODE of the original commissioner to support the National Tariff Payment System.
The NHS England document "Who Pays? Determining responsibility for NHS payments to providers" sets out a framework for establishing responsibility for commissioning and paying for a PATIENT’s care within the NHS.

- Examples to help clarify the boundaries of responsibility between commissioning ORGANISATIONS.

Where NHS England is the responsible commissioner for a specialised SERVICE, based on the NHS England Commissioner Assignment Method (CAM), one of the Specialised Commissioning Hub ORGANISATION CODES should be used depending on which Health Care Provider delivered the SERVICE, e.g. NHS Trust, Independent Sector Healthcare Provider.
The NHS England website provides a mapping list of which Health Care Providers map to which Specialised Commissioning Hub. The mapping can be found on the Organisation Data Service web pages at: Provider to commissioning hub mapping. Note: Default Code descriptions have been updated in Data Dictionary Change Notice 1934 "Default Codes Description Updates". The Data Set specifications that contain this item will be updated in the next version of the Information Standard where it is not already correct.

ORGANISATION IDENTIFIER (CDS RECIPIENT) is the same as attribute ORGANISATION IDENTIFIER.
ORGANISATION IDENTIFIER (CDS RECIPIENT) is the NHS ORGANISATION IDENTIFIER (or valid Organisation Data Service Default Code) for an ORGANISATION identified as a recipient of the commissioning data set data.
Usage:
A maximum of 7 ORGANISATION IDENTIFIERS (CDS RECIPIENT) may be submitted. The submission order does not infer the primacy of one recipient over another.
A Recipient may be an agency or SERVICE provider that carries out the receiving (and perhaps other) processes on behalf of the NHS ORGANISATION that ultimately uses the data.

Please note that the following Organisation Data Service Default Codes must not be used in the Commissioning Data Set (CDS) header because they are not default Commissioner codes:
- Q99 - High Level Health Geography/Primary Care Organisation of Residence Not Known
- a valid ORGANISATION IDENTIFIER (RESIDENCE RESPONSIBILITY) must be reported where a recipient of commissioning data is specified
- X98 - Primary Care Organisation Not Applicable (Overseas Visitors).
